Description

Tadalafil is a PDE5 inhibitor similar to Sildenafil but offers a significantly longer duration of action, earning it the nickname “the weekend pill.” This extended efficacy allows for more spontaneity. In addition to treating ED, Tadalafil is also approved for daily use to alleviate the urinary symptoms associated with an enlarged prostate (BPH), such as difficulty starting urination, weak stream, and frequent or urgent urination.

How it Works

Like Sildenafil, Tadalafil inhibits phosphodiesterase type 5 (PDE5). By preventing the breakdown of cGMP, it enhances the effects of nitric oxide released during sexual stimulation, leading to smooth muscle relaxation and increased blood flow to the penis, facilitating an erection. Its mechanism for BPH involves PDE5 inhibition leading to relaxation of smooth muscle in the prostate and bladder.

Usage and Safety

Tadalafil can be taken orally as needed, typically at least 30 minutes before sexual activity, or as a lower daily dose for continuous ED treatment or BPH management. The effects can last up to 36 hours. Common side effects include headache, dyspepsia (indigestion), back pain, myalgia (muscle pain), nasal congestion, and flushing. These side effects are usually mild to moderate and transient. Tadalafil is contraindicated with the use of nitrates and should be used with caution in patients with significant cardiovascular disease, or severe liver or kidney problems.
